FOREVER LINKED PERMANENT JEWELRY TERMS & CONDITIONS

By scheduling or receiving any service from Forever Linked, you acknowledge and agree to the following terms and conditions. Your participation in our permanent jewelry services constitutes your acceptance of this agreement.

1. Assumption of Risk:
You acknowledge that receiving permanent jewelry from Forever Linked involves the use of specialized tools to weld a custom-fit bracelet, anklet, or other jewelry piece onto your body. While the process is generally safe, inherent risks include but are not limited to minor burns, skin irritation, allergic reactions, or discomfort.

You understand that you are responsible for notifying Forever Linked of any metal allergies or sensitivities prior to the procedure. You acknowledge that once applied, the jewelry is designed to be permanent but may be removed with scissors or wire cutters if necessary.

2. Medical Conditions & Allergies:
You confirm that you do not have any medical conditions that may interfere with the permanent jewelry process. You have disclosed any relevant allergies, including but not limited to reactions to gold, sterling silver, or other metals used in the jewelry. Forever Linked is not responsible for any allergic reactions or adverse effects related to the jewelry.

3. Minor Consent:
If the participant is under the age of 18, a parent or legal guardian must provide full consent for the minor to receive permanent jewelry. The parent or legal guardian assumes full responsibility for the minor’s participation.

4. Release of Liability:
By receiving services from Forever Linked, you release, discharge, and hold harmless Forever Linked, its owners, employees, contractors, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, injury, or harm that may occur during or after receiving the permanent jewelry service. Forever Linked is not responsible for any allergic reactions, injuries, or adverse effects related to the jewelry or procedure.

5. Photography & Social Media Release (Optional):
By receiving services, you consent to the use of photos/videos taken during your appointment for marketing and social media purposes unless otherwise stated in writing.

6. All Sales Are Final:
All sales of permanent jewelry services and products are final. No refunds, exchanges, or returns will be accepted once the service has been provided. By proceeding with Forever Linked services, you acknowledge and agree to this policy.

7. Agreement:
By proceeding with Forever Linked services, you acknowledge that you have read, understand, and agree to these Terms & Conditions, including the waiver of liability. If you do not agree to these terms, please refrain from receiving services..
